Better Charting — Head to Toe is a free practice chart. Charting all 10 systems is always free, with no login. The full teaching for each assessment piece is currently a one-time $29 Unlock. The free chart also includes four original Better Charting risk assessment tools: Pain, Fall Mobility, Skin Risk, and Wound. They are up to date with current research and created specifically to help nursing students learn how to think through a full patient safety episode. Open Better Charting once while you're online, or install it to your phone or laptop. After that, the free chart works in airplane mode. If you have the $29 Unlock, the full teaching works offline too. For patient safety, your practice chart stays on your device and is wiped automatically at the end of the clinical day, or right away if your device reconnects to the internet during clinicals.
